How to install @alfe.ai/ctrader-mcp

@alfe.ai/ctrader-mcp is published on npm as @alfe.ai/ctrader-mcp, and it depends on @modelcontextprotocol/sdk. These steps assume it runs as a local stdio server, which is the usual setup.
